Indiana — Disabled Hoosier Veteran / Purple Heart / Ex-POW plates (State Form 32584)
IDVA certification of eligibility for Disabled Hoosier Veteran, Purple Heart, and Ex-POW plates (R13 / 7-20). Mail or hand-deliver to IDVA, 777 North Meridian Street, Suite 300, Indianapolis, IN 46204-1421. Approved applications come back with an embossed seal — they cannot be faxed. Then take the sealed form to a BMV branch. Distinct from the Hoosier Veteran plate, which is DD-214 at the BMV with no IDVA form.
Bring. Form 32584. DHV: VA award letter listing each rating (mobility rules apply). Purple Heart: award proof. Ex-POW: POW documentation.
