Monthly payment to legally blind wartime veterans (or unremarried surviving spouses) living and domiciled in New York. Blindness need not be service-connected. Mail the Blind Annuity application, Report of Legal Blindness, and discharge papers to NYS DVS, 2 Empire State Plaza, 17th Floor, Albany, NY 12223.
